OCHSNER CLINIC PRECEPTORSHIP
The preceptorship at Ochsner Clinic is proving to be all that I
had hoped it would be. The operative schedule is intense, with about 10
major colorectal operations each week. There is a much larger drawing area
for problems such as ulcerative colitis, as well as cancer, diverticulitis
and functional problems. As a regional referral center many of the
problems are complex as well as many reoperative cases. The opportunity to
discuss questions and problems with five nationally recognized colorectal
surgeons (three of them are board examiners) over an extended period of
time is very rewarding. I have also been asked to co-author a chapter for
an upcoming colorectal surgery textbook. I will be returning to Bremerton,
at the end of February and look forward to helping care for your patients,
starting March 1.SACRAL NERVE STIMULATION
FOR FECAL INCONTINENCE
Dr. Margolin (Ochsner Clinic) is participating in a national
study of sacral nerve stimulation for fecal incontinence. The results so
far are very impressive. Our most recent patient stated she felt like she
had been let out of prison. She is looking forward to traveling after
implantation of the permanent generator next week. Sacral nerve
stimulation will be a great addition to the treatment of fecal
incontinence once approved by the FDA, especially those patients with
sensory nerve dysfunction. There may also be a role for patients with
pelvic floor dysfunction and outlet constipation.

TRANSRECTAL ULTRASOUND
Transrectal ultrasound is routinely used to follow up
transrectal tumor excision and as a pre-operative diagnostic study for
staging rectal cancers and to evaluate the sphincter muscles in patients
with fecal incontinence. Since I trained to do this study after my
fellowship, it is very helpful to review my technique and interpretive
skills with such experts. I can perform transrectal ultrasound at Harrison
Hospital.
PELVIC FLOOR FORUM
The next Kitsap Pelvic Floor Forum will be March 8th at the Silverdale Red
Lion, 6:30 to 8:30pm. The topic of this dinner meeting, sponsored by
Bard-Urologic, is urinary incontinence. Dr. Jim Ross MD PhD will be our
speaker. Dr. Ross is currently Director for the Center for Reproductive
Medicine & Laparoscopic Surgery in Salinas, CA. He is also Clinical
Professor, Department of Obstetrics & Gynecology, UCLA School of Medicine
in Los Angeles, CA.

MARCH IS: COLORECTAL CANCER AWARENESS MONTH
Current recommendations are colonoscopy every 10 years starting at 50 for
average risk patients. Patients with a family history of colorectal
cancer, previous polyps or inflammatory bowel disease should have
colonoscopy more often. If the family member’s cancer occurred before age
60 then the patient should start 10 years younger. |
